Practice Management on Embodia - Part 3: Billing

To be able to submit a claim on behalf of a patient, the TELUS eClaims terms and conditions dictate that you must obtain prior authorization and consent from the patient. The scope of the consent is outlined in Electronic transmission authorization and consent form provided by TELUS.

One way to get consent from the patient is by providing them with the Electronic transmission authorization and consent form, asking them to fill it out, and then retaining the signed copy in the patient file.

You can also use the Embodia questionnaires feature to obtain this consent. When the TELUS eClaims feature is enabled for your clinic, this authorization and consent form will be added to the available questionnaire templates. Under HEP > Questionnaires > For the clinic account, click on Browse available templates and then search for TELUS eClaims - Electronic transmission authorization and consent form template.

Once you create a questionnaire from this template, make sure to edit the first section by adding your provider/clinic details. It is highly recommended not to change any other sections in the consent form to keep it aligned with the original form provided by TELUS.

Once done, you will be able to send this questionnaire to the patient to complete and their response will automatically be saved in their profile on Embodia.
